Clinical team research

Our clinical research, managed by Prs. Jean-Louis Pépin and Renaud Tamisier, is mainly driven on a dedicated clinical research facility on the CHU Grenoble Alpes first floor. The research is carried out by motivated practitioners, a cohesive clinical research associates team, and a biostatistician team providing quality and professionalism.

When a patient is diagnosed with a Sleep Apnea Syndrome (SAS), he or she has the opportunity to participate in several innovating clinical trials corresponding to his or her clinical profile et interests (more than 800 patients are included in a clinical trial every year). These clinical trials are more and more often designed in partnership with the patients (learn more about the patients university) and CRSIH, a current european project, is willing to support this co-working between researchers and patients.

Main clinical investigation axis

HP2 focuses on 4 major research thematic:

1/ Sleep Apnea Syndrome and its co-morbidities characterization:

The SAS is an heterogeneous chronic disease associated with various risk factors and corresponds to various clinical presentations.
HP2 performs a complete characterization of cardiovascular, metabolic, and neuropshychic phenotypes in the patients with SAS within several cohorts or registers : Obesity Hypoventilation Syndrome (OHS), obese Obstructive Sleep Apnea Syndrom (OSA) with or without co-morbidity, Central Sleep Apnea (CSA), SAS with chronic respiratory failures, SAS with BPCO, etc.
The phenotype of interest identification (Bailly S, PLoS One 2016) contributes to the implementation of a patients with SAS personalized medicine.
This strategy takes advantage of the Grenoble's register MARS (Multimorbidity, Apnea, Respiratory failure, Sleep database ; n > 2 500), the european register ESADA, and the national cohort OSFP (whose primary investigator is Jean-Louis Pépin) 100 000 patients are prospectively watched in this cohort.
Furthermore, the massive data produced by treatment equipements generate new knowledge.

2/ The Sleep Apnea syndrome modulates the chronic diseases occurrence and evolution:

The HP2 Laboratory is a European leader in physiopathological aspects as well as intervention characterization and studies in the SAS and chronic diseases connections:

  • The SAS is associated with cardiovascular complications (arterial hypertension, arterial stiffness, heart arrhythmia, heart failure, etc.),
  • Metabolic complications (diabetes, metabolic steatosis),
  • The connection between SAS and cancer is currently being disscussed and investigated.

3/ Therapeutic innovation and its validation:

The HP2 Laboratory contributes to therapeutic innovation in many fields:

  • Pharmaceutical (vigilance stimulants)
  • Instrumental therapeutics (Continuous Positive Airway Pressure, non-invasive ventilation, mandibular advancement devices, positional treatment…)
  • Hypoglossal nerve stimulation, kinesthesic stimulation.

4/ Combinated, integrated, and multimodal patients with SAS care innovation:

This perspective brings, among others, SAS precision and personnalized medicine together (learn more about Sport4Health). This integrated care takes advantage of the Hypoxia-Exercice team integrated physiology expertise.

HP2 exploration facilities in order to reach its objectives :

  • Cardiovascular (endothelial fonction, arterial stiffness, microcirculation exploration, peronal nerve microneurography),
  • Muscular (exploration during an effort, magnetic and electric stimulations),
  • NIRS (Near Infrared Spectroscopy),
  • Healthy subject exposition (Hypoxia tent, developped in collaboration with Harvard University, Tamisier R, Eur Respir J 2011)

Ambitions

The above enontiated research domains exploration allows:

  • A better characterization of the sleep function, especially of SAS occurrence and chronic diseases evolution,
  • A SAS personalized medicine implementation,
  • The establishment of a real life integrated care using e-health and connected health,

The translational research in collaboration with the Basic Science team helps to understand the mechanisms, to discover new interest biomarkers, and new therapeutical targets.

LIFE (is MaDE of ChoiCes) : LIFE is a Cross Disciplinary Program, financed since 2016 by the IDEX. Its major issue is the health trajectories determintion approach, taking into account under-estimated but substantial factors such as healthcare access, socio-economical factors, environmental exposure and urban organization.
